Threads of Change strives to serve all underprivileged youth residing in King County who have been referred for services.
Referrals are accepted from:
Case managers from community based organizations serving youth and families (10 referrals allowed by each organization (includes all locations for each organization) per quarter.
School staff (10 referrals allowed by each school per quarter)
Community In Schools site coordinators (10 referrals allowed for all locations per quarter)
ELIGIBILITY REQUIREMENTS - ALL CONDITIONS MUST BE MET
Youth must be between the ages of 12- 18
Reside in King County
Deemed underprivileged by referring party
Cannot be in foster care
Parents/caregivers wanting to access services may contact their child’s school, community-based organization, or Community In School’s site coordinator serving their child to request a referral.
Please complete the form below. One of our staff will contact the referring party within 48 hours. The turn around time for requests is typically 2-4 weeks. This may change depending on the availability of the items requested in our inventory.
